tapered line 15
Polypharmacy usually means taking five or more regular medications; not always a problem, but is every one still needed?
Tapered Line
Tapered Line is a series of one-sentence notes on medicines, alcohol and other drugs, and mental health, for patients and the clinicians who treat them. The aim is education and advocacy: safer, more thoughtful prescribing, and better conversations about starting, changing, reducing and stopping medicines, including alongside alcohol or other drugs. Like a good taper, each note is shortened slowly and carefully, keeping what matters. New notes appear most weeks; where one has a source, or a page that says more, it links to it.
Written by Dr Basanth Kenchaiah, a general adult and addiction psychiatrist in Melbourne. These notes are general information, not individual medical advice.

tapered line 12
Optimising continuity of care could help reduce polypharmacy and inappropriate prescribing.

In a study, half of aged care residents had 8+ medicines a day; can we ensure each deserves its place to keep them safe?
tapered line 9
Medicines are added with good intentions; who decides if and when one should come off?
tapered line 8
Avoid mixing opioids, sleeping tablets and alcohol; together they can slow your breathing, even at usual doses.
tapered line 7
What does a medication mean to you, beyond what it is prescribed for?
tapered line 6
Deprescribing is the planned, supervised reducing/stopping of a medicine that may be causing harm or is no longer helping.
tapered line 5

tapered line 4
Drink no more than 10 standard drinks per week and no more than 4 standard drinks on any one day.
tapered line 3
Naloxone reverses opioid overdose; in Victoria it is free from participating pharmacies and 24-hour dispensing machines.
tapered line 2
The goal is not a shorter prescription list but a safer and more effective one.
tapered line 1
Seeking help is not a judgement on you; it is the step that matters.
